The first quest of Act IV is activated by speaking with the Archangel Tyrael, who will be found in the Pandemonium Fortress. Izual has powerful Frost Nova and melee attacks. When you defeat him, his spirit appears and speaks to you; return and speak to Tyrael, and he will reward you with 2 addtional skill points.

Activate this quest by speaking with Cain; he will tell you that now you need to destroy Mephisto's Soulstone on the Hellforge, located in the River of Flame. You may find Hadriel along your way; she is guarding the entrance to the Chaos Sanctuary, and will not let you pass until this quest is completed. The Hellforge is guarded by Hephasto the Armorer, who is extra strong, cursed, and magic resistant. After the Armorer and his minions are killed, pick up the Hellforge Hammer and destroy the Hellforge. You will be given 4 gems (2 flawless, 1 perfect, and 1 normal). Return to Cain to complete the quest.

Activate this quest by speaking to Tyrael after completing the Hellforge quest. He will tell you that you must go and destroy Diablo. You will first come up against three uniques: Lord DeSeis (extra strong, thieving--takes health potions out of your belt!), Infector of Souls (extra fast, spectral hit), and Grand Vizier of Chaos (extra strong, fire enchanted).

While you are wandering through the Chaos Sanctuary, you will find seals positioned in three corners. There are a total of 5 seals inside the Chaos Sanctuary; one is positioned alone, the others will be found in pairs. Touch the seal and it will open; then touch the other seal nearby, and one of the unique monsters will appear, surrounded by minions. After you have defeated one unique, open the next set of seals to release the next unique and defeat him.

Once you have defeated all three uniques, Diablo will appear near the pentagram. He will hit you with fire, lightning, and cold, although his lightning appears to do the most damage. Most characters will do best to use hit-and-run tactics, and have an open portal nearby. When Diablo is killed, he will drop a unique, rare, or set item. Return to the Pandemonium Fortress and speak with Tyrael to complete the quest. Your character will be granted a title for defeating Diablo and you will now have access to the additional difficulty levels of the game.
